Douglas County Health Department (DCHD) has a VERY limited # of Pfizer vaccines for 16/17 year old students and will hold a brief clinic next Tuesday, April 27 from 4-4:30 p.m. at the main DCHD office (1250 E. US Hwy. 36). Parents MUST RESERVE A SLOT by calling DCHD at 217-253-4137 option 2 and leave name, DOB, and contact information (phone # and email address).
Below is the Pfizer consent form that MUST be signed by a parent or legal guardian, as well as a screening form. Both will be required at time of vaccination. Again, doses are extremely limited, so calling to leave their name and contact info for a spot next Tuesday afternoon should be done as soon as possible.
